    It seems the the only skin getting tension is the top and sides of the shaft. The bottom side of the shaft skin that attatches to the scrotum is basically unaffected when using the tugger and leg strap. Does this matter?

    The underside, or raphe side, is the most difficult to tension and get to grow for a couple of reasons. One, its proximity to the scrotum. When you apply tension the scrotum wants to move upwards with the shaft skin. I combat this by having an over head anchor above my bed for my tugging strap and tug at a slightly upward angle off 90 degrees. You can also try tugging while sleeping with an over the shoulder rig for your strap, but be careful. Sleeping with a device without ample skin to accommodate an erection can be dangerous. Second, the raphe is where everything closes up because your DNA decides you are going to be a male. This skin here is a little thicker and needs some extra attention. Manual is also a good way to target this area. When you've been at this for a while the skin there will thin out some and be more receptive to growth.

      Personally, I don't think it matters, or it didn't seem to in my case. But because tugging is not a science, who knows what the variation among guys actually is. Anecdotally there seems to be a variation in raphe thickness out there, but I don't know that for a fact. I just went for it and it all evened out in the long run. Of course that 'long run' was 9 years plus, during which I tried various methods, but at no time did I focus on the ventral surface only, even with manual (although with manual I did put more tension on the underside than any device did). So you'll have plenty of time to try different things, and get a good result.

      Thing to remember is, if you've seen the end of an intact foreskin which overhangs (acroposthion, it's called) then you'll see that it is not even. So don't worry, plenty of time, all natural result, because it's skin, biological tissue, and it does all the adding-work.

        To restrain the underside (ventral) skin when you tug, you could:
        - Use around-the-waist strapping. The other end of the strap clips to a bandanna looped over the base of the penis. The bandanna limits skin displacement somewhat.
        - Use a traction rig http://tlctugger.com/product/sizegenetics-extender-set/ as your source of tension. The rig encircles the base and limits scrotal and pubic skin mobility.
        - Tug downward toward the knee, routing the shaft OVER the waistband of your underwear, as this post https://foreskinrestoration.vbulleti...1742#post11742 illustrates. Some trial and error will help you find the right fit and style to use for comfort.
        - Tug UPWARD over the shoulder when you get the chance. Many guys exploit their sleep hours. I use a spare pillow under my knees to help me avoid flopping around so I can stay flat on my back for consistent tension.
